Job Summary
The Care Coordinator is responsible for fostering trusting connections with individuals served, conducting assessments, and developing care plans. The role involves collaboration with healthcare professionals and advocating for clients' rights while ensuring compliance with agency guidelines.
Required Qualifications
- A bachelor's degree in social work, psychology, sociology, or a closely related field
- 1 year of service planning experience
- Active MN license for those with a Master's Degree in Social Work
- Valid MN driver's license
- Acceptable driving record
- Proof of auto insurance
- Must pass all background checks
- Ability to pass the screening required by the Federal Human Services Office of Inspector General
Desired Qualifications
- Knowledge of MA, Medicare, and MN Healthcare programs
- Familiarity with community resources and providers related to aging and disabled populations
- Strong organizational skills and attention to detail
- Proficiency in computer basics and familiarity with the Microsoft Office suite
- Certified secondary language proficiency
