Care Coordinator/Life Skills Instructor- Home & Community Based

Job Summary
Teach adults with serious mental illness life skills and community connection strategies using a strengths-based, recovery-focused approach. Coordinate care plans and transport clients to appointments, utilizing a private vehicle to move staff and patients as required. Connect individuals to housing and employment resources addressing social determinants of health within Central Indiana counties. Requires a Bachelor's degree in social work or counseling, two years of related experience, and a valid driver's license with clean record. Includes a $1,000 hiring bonus and mandatory drug screening, TB testing, and background checks.
Required Qualifications
- Bachelor's degree in social work, psychology, counseling or a directly-related human service field from a college/university accredited by the US Department of Education
- Two (2) years directly related experience in past professional and/or nonprofessional roles
- Valid driver's license
- Current automobile insurance
- Driving record that meets the guidelines and requirements of the organization
- Ability to use private car to transport self and others
- Flu shot
- Fully vaccinated against COVID19
