Job Summary
Care Coordinator at ROWI Teen & Parent Wellness Centers coordinates individualized care for ROWI clients, including orientation, transportation, doctor visits, and scheduling. Responsibilities include monitoring client safety and developing safety plans, maintaining clinical documentation per ROWI and Joint Commission standards, communicating essential safety and progress information to the interdisciplinary team, delivering therapy sessions aligned with the designated level of care, engaging with referral sources and outpatient professionals post-admission, collaborating on discharge planning, facilitating group and multi-family therapy sessions, contributing to team meetings, and serving as a positive role model to foster a nurturing and professional environment.
Required Qualifications
- Master’s Degree in Marriage and Family Therapy, Counseling, Psychology, or Social Work from an accredited institution
- Must be licensed or eligible as an MFT, Clinical Psychologist, or Social Worker
- 1 year of experience in the mental health field
- Valid driver’s license and eligibility for insurance on agency vehicles
- Current First-Aid & CPR Certifications
- Satisfactory criminal background check
