Job Summary
Care Coordinator for the CHOICES Program conducts pre and post screenings to determine appropriate levels of care in hospitals, skilled nursing facilities, and supported living facilities; educates clients and families on choices/options for care; coordinates prescreening appointments and documentation to ensure smooth transitions; liaises with hospital discharge planners and nursing home staff; travels ~75% between hospitals and facilities in the Northwest Suburbs; requires Bachelor’s degree in social work or psychology; exempt full-time role with a 37.5-hour work week and ample benefits; role includes supervision participation, documentation compliance, and collaboration with agency staff to maintain continuity of care.
Required Qualifications
- Bachelor’s degree from an accredited college or university in social work, psychology or a related discipline
- Previous experience in the provision of social services to older persons and/or persons with disabilities
- DOA Community Care Program Certification preferred
