Job Summary
Care Coordinator responsible for managing a panel of 400-450 patients with monthly outreach and 50-60 daily outbound calls to provide care plan support and health coaching. Collaborates with clinical teams, advocates for patient needs, and documents interactions in EMR while maintaining HIPAA compliance. Roles include addressing care gaps via Chronic Care Management, Behavioral Health Integration, and Remote Physiological Monitoring, with a 100% remote schedule in the United States (Kansas). Requires medical assistant certification (CMA/RMA/CCMA/NCMA/CMAC) or CPhT and at least 2 years of clinical experience. 100% remote with HIPAA-compliant home office setup; candidates must be US residents. Required Qualifications
- Must hold a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent state-issued MA-C).
- OR Certified Pharmacy Technician (CPhT)
- Minimum 2 years of clinical experience in a primary care, hospital, or specialty healthcare setting.
- Experience with Electronic Medical Records (EMRs) and strong computer navigation skills.
- Proficiency in multiple technology platforms and ability to navigate complex systems.
- HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability.
- Must be legally authorized to work in the United States.
