Job Summary
Care Coordinators link adults and children with chronic behavioral health and medical conditions to the services they need to stay healthy; responsibilities include assessing risk, developing individualized care plans (ICP), providing required care management services, tracking referrals using HIT, ensuring access to services (medical, behavioral health, housing, educational services, entitlements), conducting outreach, responding to information requests, reassessing needs, completing documentation, and collaborating with team members. The role involves field work across New York City, travel to member homes and community settings, and carrying an agency-provided cell phone.
Required Qualifications
- Bachelor's degree in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing; OR NYS teacher's certification; OR NYS licensure and registration as a Registered Nurse with a bachelor's degree plus two years of direct service/case management experience; OR Bachelor's/Associate/HS Diploma in another discipline plus five years' experience in an applicable population.
