Job Summary
Care Coordinators link adults and children with chronic behavioral health and medical conditions to needed services, assessing risk, developing person-centered care plans, coordinating care, tracking referrals, and educating members to access medical, social, and community services. The role involves field work, travel within New York City, and collaboration with family, caregivers, and other providers to ensure engagement with services, crisis response, and ongoing documentation using an electronic health record. The position requires strong communication, cultural competence, and the ability to work both independently and as part of an interdisciplinary team.
Required Qualifications
- A bachelor’s degree in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing; OR NYS teacher’s certification with bachelor’s degree; OR NYS licensure and registration as a Registered Nurse with a bachelor’s degree plus two years of relevant direct services/case management experience; OR a Bachelor’s or Associate’s degree or High School Diploma/GED in another discipline plus five years of applicable experience; Specific experience with the target population may be required to work with Children, Health Home Plus or Adult Home Plus members.
