Job Summary
Care Coordinator for Segue Health in Jackson, TN coordinates post-discharge transitional care management (TCM) for patients; works with referring facilities to identify eligible patients, visits referrals in facilities to assess needs post discharge, logs referrals and discharges, creates charts in EMR and uploads face sheet, H&P, and DC Summary, conducts post-discharge calls within 24 hours, and schedules patients for in-home visits. The role requires experience in the medical field, proficiency with Microsoft Excel and EMR/EHR software, and reliable transportation to and from the hospital. Attention to detail and organizational skills are emphasized; vaccinations per hospital standards are required. Our team emphasizes compassionate, Christ-centered care and coordination of care from hospital to home.
Required Qualifications
- Must have experience in the medical field
