Job Summary
Coordinate behavioral health care management services for youth and adults through mobile outreach, partnering with medical teams and care professionals to implement treatment and safety plans. Monitor client progress, evaluate service needs, and provide natural supports by identifying community resources while advocating for individuals within the program and with external agencies. Complete required documentation including medical records, quarterly reviews, and progress notes, and actively participate in supervision, teaming, and clinical rounds. Transport clients using a valid driver's license and operational vehicle.
Required Qualifications
- High school diploma or equivalent
- Two years experience working within an outpatient, crisis and/or medical settings
- Ability to use an Electronic Health Record to document medical necessary clinical services
- Excellent written and verbal communication skills
- Ability to thrive in a fast-paced, team environment
- Strong organizational skills and ability to think creatively
- Must hold a valid driver's license
- Have access to an operational and insured vehicle
- Be willing to use it to transport clients
- Ability to provide mobile services required
Desired Qualifications
- Demonstrated understanding of and competence in serving culturally diverse populations
- Bilingual in Spanish
