Care Coordinator

Job Summary
Scheduling and rescheduling patient appointments while answering phone calls and managing daily cash collection. Engage patients and families by orienting them to the care plan, explaining treatments and medications, and initiating care coordination. Monitor delivery of care through patient rounds, document progress, and intervene when deviations occur. Review EHR data to analyze utilization trends, create interdisciplinary care plans, and coordinate with providers to resolve issues affecting patient flow. Liaise with insurance carriers and hospitals to negotiate scheduling for tests and procedures, ensuring continuity of care and adherence to organizational policies.
Required Qualifications
- High School Diploma or equivalent
- Ability to speak a Spanish and English fluently
- Must be able to follow up and ensure policies and procedures are followed
- Must be able to work flexible schedules and locations
- This position is onsite
- Requires travel between Willowbrook, TX and Spring, TX
Desired Qualifications
- Bachelor's degree
- 3-5 years' experience in clinical or community resource settings
- Care coordination and/or case management experience
- Advanced knowledge of Excel
- Advanced knowledge of EHR
- Experience with health IT systems and reports
- Local knowledge about and connections to community health care and social welfare resources
