Job Summary
Care Coordinator III supports Transitional Age Youth (18-25) in Orangewood Foundation’s Young Adult Court program, delivering trauma-informed case management, crisis intervention, and resource/navigation services across community, office, and court settings. The role uses motivational interviewing to engage participants through an 18- to 24-month collaborative process, documents progress for the team and court, and coordinates services with a multidisciplinary group. Requires a Bachelor’s degree in a related field and 3+ years of relevant direct-service experience (or 1 year with substantial lived experience), plus a valid California driver’s license and DMV clearance. Flexible schedule with availability for evenings/weekends; Plum.io assessment required for consideration. Essential values include equity, confidentiality, professionalism, and a supportive environment for participants.
Required Qualifications
- Bachelor’s degree in Criminal Justice, Human Services, Sociology, Social Work, Education, Psychology, or a related field required
- 3+ years of experience in a mental health, social services, collaborative court, or community services organization providing direct services to the target population, OR 1 year of direct service experience combined with significant lived experience relevant to the population served
- Experience working with transitional-age youth, particularly those involved in the criminal justice system
- Strong understanding of trauma-informed care, harm reduction, motivational interviewing, coaching, and mentoring practices
- Excellent verbal and written communication, interpersonal, and relationship-building skills
- Ability to work collaboratively within a multidisciplinary team and coordinate services effectively
- Demonstrated ability to work sensitively and effectively with youth impacted by trauma
- Strong organizational, documentation, and data entry skills, including proficiency with electronic database systems and standard computer applications
- Ability to identify opportunities and challenges, think critically and strategically, and make sound and timely decisions under pressure
- Ability to maintain confidentiality, accept and implement feedback, and demonstrate professionalism and accountability
- Commitment to fostering an inclusive, equitable, and supportive environment for all participants
- Possess a valid California driver’s license and proof of automobile insurance. Position requires DMV clearance
- Pre-employment screenings include criminal background check, FBI/DOJ fingerprinting, TB test, and drug screen
