Job Summary
Care Coordinator II, Special Projects (NCSH Residential) at HealthRIGHT 360 provides comprehensive support and advocacy for clients in a gender-responsive residential program. Responsibilities include direct client interactions, individual and group sessions, crisis intervention, development of treatment plans, coordination with Child Welfare Services, linkage to internal and external resources, documentation and compliance, and participation in collaborative meetings. The role emphasizes trauma-informed care, assisting clients with families, managing caseloads, and maintaining accurate records in compliance with HIPAA and funder standards. Required Qualifications
- High school diploma or equivalent required
- Bachelor’s degree in Health and Human Services, Social Work, Psychology, or related field preferred
- Registration or Certification as a Substance Use Disorder Counselor from an accredited California agency (CCAPP, CAADE, CADTP)
- Valid First Aid and CPR certification or ability to obtain within 30 days of hire
- Valid California Driver’s License and access to registered and insured transportation
- Experience carrying a caseload
- At least one year case management experience
- Must complete background check and livescan clearance
