Job Summary
Care Coordinator II responsible for coordinating care for patients with chronic conditions and behavioral health needs at point of care; participates in morning huddles, identifies gaps in care, provides brief interventions at point of care, links patients to behavioral health and community resources, coordinates care transitions from ER/hospital to home, and supports quality improvement initiatives within a Sea Mar Clinic setting. Requires knowledge of chronic disease management, motivational interviewing, patient-centered medical home concepts, and strong communication with diverse populations. LPN with ambulatory care experience or BA/BS/BSW in health-related field or 4 years equivalent experience; BLS CPR within 90 days; Spanish preferred; NCQA certification a plus; requires WA driver’s license; full-time, Seattle, WA; hourly rate $23.31.
Required Qualifications
- LPN with experience in ambulatory care and/or BA/BS/BSW in health-related field with one year of experience working in community health, or, 4 years of equivalent experience
- This position must obtain Basic Life Support (BLS) CPR within 90 days of hire date and is required to maintain current BLS CPR throughout employment
- NCQA Certification is a plus
- Valid WA State Driver’s License and proof of liability insurance
Desired Qualifications
- LPN or BA/BS/BSW in health-related field or 4 years of equivalent experience
- Basic Life Support (BLS) CPR within 90 days of hire and maintained
- NCQA Certification is a plus
- Spanish language skills strongly preferred
