Job Summary
Care Coordinator II at Sea Mar Community Health Centers coordinates care for patients with chronic conditions and behavioral health needs within the Clinical Care Team. Responsibilities include participating in daily huddles, identifying gaps in care, providing brief interventions, coordinating care transitions after ER/hospital discharge, connecting patients to medical, behavioral health, dental, and community resources, tracking patient progress in the EHR, supporting quality improvement, and assisting with the 340B medication assistance program as site dependent. Requires LPN ambulatory care experience or a BA/BS/BSW in a health-related field with 1 year of community health experience (or 4 years of equivalent experience), strong communication and organizational skills, and bilingual English/Spanish strongly preferred. Valid WA driver’s license and proof of insurance, COVID-19 vaccination, and BLS CPR within 90 days of hire are required.
Required Qualifications
- LPN (ambulatory care experience preferred)
- BA/BS/BSW in health-related field + 1 year community health experience OR 4 years equivalent experience
- Strong communication, organization, and patient engagement skills
- Knowledge of chronic disease and behavioral health preferred
- Bilingual English/Spanish strongly preferred
- Valid WA driver’s license and proof of insurance required
- COVID-19 vaccination required
- Must obtain BLS CPR within 90 days of hire
