Job Summary
Coordinate multiple aspects of youth and family care by assessing needs, developing individual service plans, and implementing strength-based interventions including home visits and community resource coordination. Deliver psychoeducational training and skill development sessions while providing transportation to medical and behavioral health appointments. Maintain comprehensive documentation and communicate progress to the multi-disciplinary treatment team. Lead projects or mentor staff as assigned, while managing on-call responsibilities for the Foster Care program. Requires a valid NYS driver's license and ability to work variable schedules including evenings and weekends.
Required Qualifications
- Bachelor's degree
- Minimum 18 months of relevant experience
- Therapeutic Foster Care program: Master's degree in Social Work or a related human services field, OR Bachelor's degree in Social Work or related human services field with 2 years of related experience
- Unrestricted, valid NYS driver's license for minimum of 1 year
- Clean driving record
- Minimum insurance coverage that meets agency standards
- Ability to understand and adhere to all federal, state, and local statutes, regulatory agency standards and Hillside policies
- Ability to de-escalate and manage crisis situations both in-person and by phone
- Ability to deliver psychoeducational training
- Ability to work collaboratively as part of a multi-disciplinary treatment team
- Ability to work through a series of issues using independent judgment
- Ability to be independent and self-directed
- Must be able to work a variable scheduling including evenings and weekends
- Sitting (up to 6 hours)
- Occasional standing (up to 2 hour)
- Occasional walking (up to 2.5 hours)
- Driving (several times a week up to 6 hours)
- Continuous balancing (up to 8 hours)
- Occasional balancing, bending, stooping, climbing, kneeling, pushing, pulling, reaching forward or down, reaching overhead, running, and twisting (up to 2.5 hours)
- Weekly lifting up to 10-15 lbs. on a daily basis
- Manual dexterity is required, including the frequent ability to grasp in both hands and continuous use of fine manipulation skills in both hands (approximately 1-2.5 hrs.)
- Occasional exposure to dust, fumes, gases, chemicals, or smoke
