Job Summary
As a Care Coordinator II, you will act as a patient advocate, coordinating continuity of care both internally and with external healthcare organizations. Key responsibilities include managing high-risk patient care, supporting patient self-management, facilitating communication among team members, conducting screenings, and promoting wellness through patient education. Necessary qualifications include a high school diploma and completion of a nursing or Medical Assisting program, along with 5 years of clinical experience. Proficiency in Microsoft Office and strong communication skills are essential.
Required Qualifications
- High school graduate with successful completion of an accredited/licensed school of nursing or Medical Assisting program
- Expectation of certification of Medical Assisting program within one year of employment
- 5 years’ experience in a clinical setting required
- Must possess a valid State of Ohio motor vehicle operator's license and insurable under Firelands auto policy
- Successful completion of a 90 day get-acquainted period
Desired Qualifications
- 1-3 years’ experience in chronic disease management, case management, and utilization management
- Proficient computer skills, including Microsoft Office (specifically Word and Excel)
- Highly organized and well-developed oral and written communication skills
