Job Summary
Conduct member outreach via telephone or home visits to discuss care plans, resources, and ongoing education. Coordinate care activities with healthcare providers, community partners, and members to ensure continuity of service and accommodate changes. Perform service assessments and screenings while documenting member records in accordance with state and regulatory requirements. Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers. Provide education on benefits and resources available. Requires a high school diploma or GED, one to two years of related experience, and a driver's license. Must reside in Arkansas.
Required Qualifications
- High School diploma or GED
- 1 – 2 years of related experience
- Bachelor's degree in social science/health-related field OR a high school diploma with at least one (1) year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients
- Driver's license
- Child and adult maltreatment check (before hire and recurring)
- Drug screen (at time of hire and recurring)
- Reside in AR or border city
- Reside in Independence, Sharp, Izard, or Stone County, Arkansas
- 30% travel

- Strong experience working with both Behavioral Health and Developmental Disabilities (BH/DD) populations
