Care Coordinator II

Conduct member outreach via telephone or home visits to engage members and discuss care plans, resources, and concerns. Coordinate care activities based on service plans while working with healthcare providers and community partners to ensure continuity of care. Perform service assessments and screenings, document member records in accordance with state and regulatory requirements, and maintain compliance with contractual guidelines. Identify member needs to make referrals to care managers and disease managers, and provide education on available benefits.
