Job Summary
Care Coordinator I role responsible for developing, documenting, evaluating, and monitoring annual Person-Centered Service Plans (PCSP) for Summit Community Care members across multiple Arkansas counties. Responsibilities include coordinating behavioral and physical health services, connecting members with community resources, conducting in-person visits quarterly (with more frequent updates for complex cases), maintaining monthly contact with families, providers, and other team members, documenting activities into EMR within 24 hours, complying with HIPAA and Medicaid rules, and supporting continuous learning and performance improvement. The role requires driving a personal automobile for visits, strong communication skills in English and/or Spanish, proficiency with Microsoft Office and EMR systems, and adherence to safety-sensitive and regulatory standards. A bachelor’s degree in a relevant field (or equivalent) is preferred, along with experience with care coordination, data management, and knowledge of Medicaid/Conflict-Free Case Management rules. The position offers a remote/mobile work setup with regular travel, and provides a comprehensive benefits package, PTO, 401(k), bonuses, and a starting hourly range of $19.80–$24.40 with final offer based on education and experience.
Required Qualifications
- Bachelor's degree in social work, psychology, sociology, another social science, or a health-related field or equivalent combinations of education and experience
- one year of beginner-level experience with Microsoft Excel, Outlook, and Word
- valid Arkansas Class D (Non-Commercial Vehicle) Driver's License before employment and ability to maintain licensure for duration of employment
- access to a properly licensed and insured automobile for reimbursable business travel
- ability to commute to/from member homes, provider offices, and meeting spaces as needed
- must not be excluded or debarred from Medicaid enrollment or related regulatory requirements
- proficiency in English and/or Spanish with diverse member populations
- ability to comply with HIPAA and Conflict-Free Case Management rules
- experience or ability to develop and document Person-Centered Service Plans (PCSP) and maintain EMR documentation
