Job Summary
Care Coordinator I participates in the Interdisciplinary Care Plan team to coordinate care for members with stable chronic conditions and others requiring care coordination. Responsibilities include assessing and reviewing plans of care, identifying gaps and trends, developing and communicating plans for authorization of services, conducting health risk assessments, facilitating transitions to home care, performing face-to-face home visits, educating providers and families, serving as member advocate, and tracking interventions to improve health outcomes. Requires HS diploma with 3 years of relevant experience, valid driver's license, and ability to travel locally; proficiency in Word/Excel/Outlook and knowledge of community resources and care coordination processes. The role emphasizes cost-effective, quality care within a healthcare system context and includes activities such as documenting plans, coordinating services, and supporting safety and well-being for members.
Required Qualifications
- HS Diploma & 3 years experience
- Must have a valid driver license, clean driving record, and able to travel locally
- Experience in utilization management, quality assurance, home care, community health, long term care, medical assistant or occupational health required
- Proficiency in Microsoft Word, Excel and Outlook required
