Job Summary
Care Coordinator for Hospital at Home and Care After Discharge seeking RN to assess and identify qualifying patients for Hospital at Home, coordinate onboarding into the program, and facilitate admission activities with the care team. Responsibilities include collaborating with a multidisciplinary team to coordinate patient logistics and clinical care, educating patients and families on telehealth tools and equipment, and ensuring HIPAA/privacy compliance. Requires RN licensure in New Jersey, CPR/BLS certification, and a minimum of 2 years of acute care nursing experience; BSN preferred. On-site role located in Pennsauken, NJ.
Required Qualifications
- Graduate of an accredited nursing program
- Current State of New Jersey nursing license to practice as a registered nurse
- Current CPR certification and BLS
- Minimum of 2 years of acute care nursing experience
