Job Summary
Care Designer (Care Coordinator) reports to the Franchise Partner and is responsible for day-to-day client care management and staff supervision, ensuring compliance with operating policies. Responsibilities include developing and updating individual care plans, conducting client care assessments, coordinating with families and primary physicians, teaching self-care, training new caregivers, supervising caregivers, maintaining client documentation (Procura), coordinating recruitment and scheduling, and supporting quality assurance and risk management. The role involves on-call duties, community representation, and collaboration with partners to identify care needs and improve service delivery. Requires three to five years in home care, prior management experience, CPR and First Aid certification, and a valid driver’s license with car insurance.
