Job Summary
Coordinate patient transitions from inpatient psychiatric units, hospitals, and residential programs back into community care. Collaborate with providers, nurses, and community partners to align treatment plans and review medications for patient safety. Educate and support patients to ensure they understand their care plans and feel confident moving forward. Build relationships across healthcare systems to strengthen mental health care in the region. Work Monday through Friday from 8am to 5pm in Hays, Kansas.
Required Qualifications
- Current Registered Nurse (RN) licensure from the Kansas State Board of Nursing
Desired Qualifications
- Bachelor of Science in Nursing (BSN)
- experience in psychiatric/mental health nursing, or physician office or outpatient practice
- experience in care coordination or case management
- Strong interpersonal and collaboration skills when working with multidisciplinary teams and external providers
- Ability to communicate effectively with patients and families about treatment plans and care transitions
- Demonstrated proficiency with technology including computers, electronic records, and common office equipment
- Commitment to continuous learning and quality improvement, particularly in mental health care and clinical processes
