Care Coordinator

Care Coordinator at Gastro Health (full-time).Responsible for comprehensive care coordination including managing patient care and coordinating treatment plans. Duties include handling administrative tasks for physicians, acting as liaison for patient medical care, streamlining patient-physician communications, providing medical literature and clinical instructions, scheduling procedures and tests, obtaining necessary authorizations, coordinating follow-ups and recalls, processing prescriptions and lab results, and managing electronic medical records. Must be a multi-tasker with strong communication, organization, and team skills; bilingual in English/Spanish required. High school diploma or GED; 2+ years medical assistant experience preferred.
