Care Coordinator for LTSS Waiver in Charlottesville, VA

Job Summary
Conduct health risk assessments for Medicaid members receiving LTSS waiver services through in-person visits and telephonic calls, performing up to three to four face-to-face member visits weekly. Manage casework duties and provide case management services to patients, families, and caregivers while developing and monitoring multidisciplinary collaboration of services. Assist adults with personal and environmental difficulties related to medical conditions, including terminal illnesses. This role requires residing in Charlottesville or within a 30-60 minute drive east or southeast, working full-time hours from 8am to 5pm EST, Monday through Friday.
Required Qualifications
- Reside in Charlottesville or within a 30-60 minute drive east or southeast of Charlottesville, including areas such as Keswick, Scottsville, Palmyra, and surrounding communities
- Possess a degree in Health & Human Services or one of the following related Fields: Art Therapy, Behavioral Sciences, Child Development, Cognitive Sciences, Community Mental Health Counseling (MH, Vocational, Pastoral, etc.), Counselor Education, Early Childhood Development, Educational Psychology, Gerontology, Healthcare Administration, Human Development, Human Services, Marriage and Family Therapy, Music Therapy, Nursing, Pharmacy, Psychiatric Rehabilitation, Psychology, Rehabilitation Coun...
- All degrees must be from schools that are listed as accredited on the U.S. Department of Education College Accreditation database
- Degrees that have been obtained from schools outside the United States will be reviewed individually
- Long Term Care experience: 1 year REQUIRED
