Job Summary
The Care Coordinator, FCFC provides assessment, care coordination, and referrals to youth and families, focusing on stability in relationships, housing, and meaningful activity. The role builds enduring relationships with those served, serves as the central contact point in the team, and coordinates care and services for a caseload of 10-20 youth/families, dedicating at least 50% of time to direct service. Responsibilities include developing and implementing comprehensive plans of care, facilitating team meetings, coordinating services through natural and community supports, advocating for individuals, and ensuring ongoing communication and documentation to support outcomes and regulatory compliance. Requires strong collaboration, assessment, planning, and outreach skills, with experience in family systems, case management, and related areas; supervisory and regulatory compliance knowledge is included through required trainings and clearances.
Required Qualifications
- Associate’s degree in a human services field
- Valid driver’s license with acceptable driving record; daily access to private insured vehicle during work hours
- Minimum of one year of experience in family systems, community systems and resources, case management, child and family counseling/therapy, child protection or child development; ability to navigate state/local child serving systems
- Computer literacy skills
- State Police clearance
- FBI clearance
- Child Abuse clearances
- Mandated Reporter-Recognizing and Reporting Child Abuse training certificate and/or statement
