Job Summary
Care Coordinator responsibilities include performing administrative, customer service, financial and reception duties, scheduling and coordinating initial assessments and referrals for patients with substance use disorders, ensuring patient confidentiality, assisting with intake paperwork, maintaining records and coordinating with Behavioral Health Therapists, Social Workers, and Primary Care Providers in a team-based approach to wellness and recovery. Requires AA degree with strong computer skills (MS Word/Excel), bilingual English/Spanish preferred, and ability to work within HIPAA/42 CFR Part 2 guidelines; role involves outreach to referral sources, crisis intervention support, and coordination of evaluations and services.
Required Qualifications
- AA degree required
- BA degree preferred
- State Certified Interpreter preferred
- Experience in administrative/intake support in healthcare or SUD settings preferred
- Bilingual in English/Spanish preferred
