Care Coordinator, Embedded (Wayne / Florham Park, NJ)

Full-time Care Coordinator, Embedded at Ennoble Care, working in a senior living facility in Wayne and Florham Park, NJ. The role partners with clinicians, staff, and patients to create and manage individualized care plans, perform care coordination and documentation using Ennoble Care’s electronic medical record system, and provide ongoing communication with patients, families, and care teams to achieve positive outcomes. Responsibilities include developing care plans, frequent patient contact for care coordination and program compliance, documenting client communications, coordinating with Primary Care Clinician or designee, updating the team on issues, collaborating across organizational areas, utilizing technology (secure email, EMR, etc.), maintaining documentation protocols, delivering excellent customer service, supporting team members, and operating office equipment. Qualifications emphasize strong phone communication, compassion, note-taking, multitasking, adaptability to a fast-paced environment, proficiency with Outlook and MS Office, healthcare terminology knowledge, and readiness to pass a drug screen and background check.
