Job Summary
Complete individualized patient care plans and perform care management and care coordination services using Ennoble Care's electronic medical record system. Maintain frequent contact with patients to provide support, manage compliance with care programs, and document all client communications accurately. Communicate with Primary Care Clinicians regarding emergent needs and keep Team Supervisors informed of care plan issues. Coordinate scheduling and support for caregivers and families while adhering to documentation protocols and best practices for daily work logs. Utilize office equipment including computers, telephones, and copy machines to meet patient and family needs. Work full-time, Monday through Friday, daytime hours at the senior living facility in Hillsborough, NJ.
Required Qualifications
- Must be comfortable with speaking on the phone for large amounts of the day
- Must be compassionate and empathetic towards our patients, always demonstrating exceptional customer service
- Ability to take accurate notes to document each task in a timely manner
- Ability to multitask between different patients and workstreams while remaining organized and efficient with time
- Ability to thrive in a fast-paced environment
- Must be able to work full-time, Monday through Friday, daytime hours, in our client's senior living facility in Hillsborough, NJ
- Must be proficient in using a computer, including Outlook and other Microsoft Office programs
- Knowledge of basic healthcare terms, conditions, roles, and basic care principles
- Must be able to pass a drug screen
- Must have a positive attitude
- Must be a team player
- Must be willing to learn new skills on a continuous basis
