Job Summary
Coordinate care and services among physical, behavioral, dental, and social systems for Managed Care Plan (MCP) members, serving as the Enhanced Care Management Point of Contact. Maintain a caseload by conducting in-person outreach, accompanying members to office visits, arranging transportation, and scheduling appointments with contract providers. Advocate on behalf of members with healthcare professionals while monitoring treatment adherence and medication management. Extend health promotion training and connect clients to necessary social services, ensuring they receive the right care at the right time to live successfully in their communities.
Required Qualifications
- High school graduation or accredited GED program
- Less than 2 years varied experience as a provider of mental health services
- California driver's license
- Current vehicle insurance/registration
- Reliable means of transportation capable of passing vehicle safety inspection if more than five years old excluding all modes of two-wheeled transport inclusive of bicycles, mopeds and motorcycles
- Monday – Friday, 8:00 am - 4:30 pm schedule
Desired Qualifications
- Graduation from a four (4) year college or university with a bachelor's degree in Social Work or related field
- Four (4) years varied experience as a provider of direct mental health services
- Meeting other Mental Health Rehabilitation Specialist qualifications
