Job Summary

Coordinate patients' healthcare by acting as the first point of contact, managing clinical calls, and producing personalized care plans that integrate identified needs. Direct liaison with multiple agencies, social prescribing link workers, and clinicians to ensure timely referrals, monitor progress, and support national screening and immunisation programmes. Proactively identify cohorts for shared decision-making, assist with self-management education, and facilitate access to personal health budgets. Manage administrative duties including data entry, document scanning, prescription handling, and responding to queries from hospitals and local providers. Maintain operational systems, support performance dashboards, and participate in audits and team meetings.

  • Ability to handle prescriptions accurately and efficiently following protocol
  • Ability to monitor incoming electronic test results
  • Ability to perform data entry of new and temporary registrations and relevant patient information
  • Ability to scan all patient information in computerised records
  • Ability to deal with referrals to primary and secondary care
  • Ability to handle action tasks set by clinicians via clinical systems and email
