Job Summary
Care Coordinator for DaVita Integrated Kidney Care (IKC) coordinating care for patients with complex chronic conditions in a remote role with potential hybrid transition. Responsibilities include coordinating access to resources/programs, handling referrals and provider appointments, performing non-clinical screenings under supervision, assisting clinical team with patient progression through the continuum of care, identifying resources and enrolling patients in programs, maintaining accurate confidential documentation, and communicating effectively with patients and care team. Requires a high school diploma or GED plus 2+ years in a professional office setting; strong verbal communication; proficiency in Microsoft Office (Excel, Outlook); familiarity with Electronic Medical Records; ability to work autonomously in a home office while collaborating with nurses, clinics, health plans, and Village resources.
Required Qualifications
- High school diploma or GED required
- 2+ years of experience in a professional office environment
- Ability to handle confidential information with discretion and sound judgment
- Strong verbal communication skills
- Proficiency in Microsoft Office, Excel, and Outlook
- Prior experience with Electronic Medical Records preferred
- Commitment to service excellence and patient satisfaction
- Ability to work autonomously while collaborating effectively with the team
- CNA or MA experience is a plus
- Experience coordinating care (referrals, care management, etc.)
- Familiarity navigating insurance payers
- Prior medical office environment experience
