Job Summary
Care Coordinator at DaVita Integrated Kidney Care (IKC) focused on coordinating care for patients with chronic conditions. You’ll work closely with Nurse Practitioners and RN Case Managers to facilitate resources, referrals, and provider appointments, perform outbound and inbound calls, complete non-clinical screenings, assist with moving patients through the continuum of care, identify eligible resources, maintain accurate documentation, and communicate professionally with patients. Requires a high school diploma or GED and 2+ years in a professional office setting; proficiency in Microsoft Office; EMR experience preferred; ability to work autonomously while collaborating with the care team; strong communication and confidentiality skills. The role is remote with potential future hybrid-remote transition and a schedule Mon–Fri, no nights/weekends/on-call.
Required Qualifications
- High school diploma or GED required
- 2+ years of experience in a professional office environment
- Strong verbal communication skills
- Proficiency in Microsoft Office (Excel, Outlook)
- Prior experience with Electronic Medical Records preferred
- Ability to handle confidential information with discretion
- Ability to work autonomously while collaborating with a team
