Job Summary
Coordinate care for patients by ensuring access to eligible resources, programs, and referrals while facilitating provider appointments. Perform outbound and inbound calls for patients, providers, and Village resources, and complete non-clinical screenings and pathways under Manager guidance. Collaborate with Integrated Care Nurses, dialysis clinics, and health plans to help members enroll in programs, and maintain accurate, confidential documentation. Act as a key player on the care team, working closely with Nurse Practitioners and RN Case Managers to keep the patient journey moving smoothly. Requires commuting to Davita clinics 3-4 times weekly with a Monday–Friday schedule starting between 7:00–8:00 AM.
Required Qualifications
- High school diploma or GED
- 2+ years of experience in a professional office environment
- Ability to handle confidential information with discretion and sound judgment
- Strong verbal communication skills
- Proficiency in Microsoft Office, Excel, and Outlook
- Reliable transportation
- Must provide flexibility as needed to meet patient and clinic needs
- Commitment to service excellence and patient satisfaction
- Ability to work autonomously while collaborating effectively with the team
- Commute to Davita clinics as required 3-4 times per week
- Full-Time, Monday–Friday schedule
- Daily start time between 7:00–8:00 AM
- Must include a quiet, private space with high-speed internet (when working from home)
Desired Qualifications
- Prior experience with Electronic Medical Records
- 1–3 years clinical medical office experience
- CNA or MA experience
- Experience coordinating care (referrals, care management, etc.)
- Familiarity navigating insurance payers
- Experience in direct patient care or support
- Prior medical office environment experience
