Job Summary
Coordinate referrals from hospitals, correctional facilities, and community partners to ensure timely engagement and appropriate program placement. Support organizational access standards, including CCBHC 7-day access requirements, by assisting individuals with navigating Medicaid, insurance enrollment, and other benefits. Facilitate transitions of care for those discharging from jail, inpatient psychiatric facilities, and residential settings while overcoming barriers related to transportation, identification, and documentation. Conduct outreach and follow-up activities to support successful connection to services, document care coordination efforts in accordance with agency standards, and monitor referral outcomes and access metrics. Maintain working knowledge of community resources, entitlement programs, and eligibility requirements while utilizing trauma-informed care, motivational interviewing, and person-centered approaches.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice or other related degree
- Active, valid driver's license
- Insurance Navigator certification for both the State of Indiana and the Federal Government
- Pass required background checks including county/state checks, fingerprinting, CPS check, sex offender registry check, and drug screen
