Care Coordinator - Community Support Services

Job Summary

Coordinate referrals from hospitals, correctional facilities, and community partners to ensure timely engagement and appropriate program placement. Support organizational access standards, including CCBHC 7-day access requirements, and assist individuals with navigating Medicaid, insurance enrollment, and reinstatement processes. Facilitate transitions of care for discharges from jail, inpatient facilities, and residential settings while overcoming barriers related to transportation, identification, and benefits. Conduct outreach and follow-up activities to support successful connection to services, document care coordination efforts in accordance with agency standards, and monitor referral outcomes and access metrics. Maintain working knowledge of community resources and eligibility requirements while utilizing trauma-informed care and person-centered approaches. Obtain and maintain Insurance Navigator certification within six months of hire.

Required Qualifications

  • Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice or other related degree
  • Active, valid driver's license
  • Insurance Navigator certification for both the State of Indiana and the Federal Government
  • Pass required background checks including county/state checks, fingerprinting, CPS check, sex offender registry check, and drug screen
