Care Coordinator

Coordinate hospice services by responding to referrals, evaluating patient desires, and meeting with families to discuss goals and explain services. Complete required paperwork, facilitate patient discharges by working with facility and hospital staff, and educate personnel on pending patients. Provide daily feedback to admission staff, attend routine meetings, and deliver in-service training to reinforce the organization as the provider of choice. Build long-term relationships at each facility to increase hospital referrals through customer service.
