Job Summary

Care Coordinators at Community Care Network coordinate care across mental health, substance use, and physical health services with person-centered and family-centered treatment goals. They facilitate access to social services, housing, education, and employment resources; collaborate with justice and child-welfare systems; conduct structured interviews, administer standardized assessments, and enter data into systems; coordinate with a multidisciplinary team to ensure holistic care; advocate for clients to navigate systems and access benefits, including Medicaid; maintain documentation and participate in ongoing reporting; pursue Vermont Insurance Assister Certification to assist clients with insurance navigation; and perform related duties as assigned. The role requires collaboration, problem-solving, compassionate communication, and the ability to manage protective health information within regulatory guidelines.

Required Qualifications

  • High School Diploma or GED required
  • Experience in mental health or substance use treatment settings, preferably in a care coordination role
  • Experience managing protected health information
  • Strong knowledge of community resources
  • Effective communication skills, both written and verbal
  • Excellent interpersonal and organizational skills
  • Proficiency with computers, Microsoft Office products, and electronic health records systems
  • Flexibility, dependability, and ability to work independently
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont
  • Successful completion of a criminal history background check after hire
  • Assist clients and families in determining eligibility for internal programs, community resources, and external benefit programs, and provide support throughout the application and enrollment process as appropriate
  • Obtain and maintain State of Vermont Insurance Assister Certification through Vermont Health Connect to support clients and families in accessing, understanding, and navigating health insurance coverage and related benefits
