Job Summary
Care Coordinator – Clinics coordinates care with clinical teams to ensure continuity of services, links consumers to community resources, identifies barriers to care, and assists with implementing supportive treatment plans. Emphasizes collaboration with clinical teams and linking families to resources; Spanish/English bilingual skills preferred.
Required Qualifications
- Bachelor’s degree in human services related field (e.g., Social Work, Education, Psychology, Sociology)
- At least one year of satisfactory work experience with individuals with intellectual/developmental disabilities, mental illness or substance use disorders
- Thorough working knowledge of professional ethics and standards of practice
- Computer skills including word processing, spreadsheets, email, database management, and data entry
- Demonstrated experience communicating clearly and concisely in stressful situations
- Excellent communication skills, both verbal and written
- Excellent customer service skills
- Ability to work effectively in a multicultural environment and bridge language/cultural barriers to Network180 services within diverse communities
