Job Summary
Care Coordinator acts as the primary consumer advocate for the assigned program, coordinates services delivered to the consumer and family through internal and external referrals, administers assessments, develops and reviews monthly Service Plans with the consumer and family, documents clinical services per licensing and Medicaid standards, conducts individual case management, provides vocational education and skills building, coordinates external appointments and transportation, and links consumers to needed resources while working with the treatment team and external providers. Knowledge of HIV/VH/STD risks, safer sex practices, substance use prevention, and ability to communicate effectively with adults 18+ to motivate behavior change are essential. Requires a Bachelor's degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related field and at least two years of supervised case management experience.
Required Qualifications
- Education: Graduation from an accredited bachelors program with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related field is required
- Two years of previously supervised work experience in providing case management services is preferred
