Job Summary
Conduct in-person, community-based child and family team meetings and review IMCANS for youth and families with high needs. Provide assertive outreach, advocacy, support, and follow-up to assigned clients while collaborating with service teams to implement personal growth plans. Monitor service effectiveness, document outreach attempts, and report Critical Incidents per policy. Assist clients in building natural support teams and integrate community resources into the service delivery process. Present relevant information for quality assurance and care planning changes. Full-time role with pay range $16.50--$23.10 and travel required across Madison, Calhoun, Jersey, Greene, Macoupin, Montgomery, and Bond counties.
Required Qualifications
- Bachelor's Degree in a human services-related field
- Associate's Degree in human services plus at least three years of hands-on experience in mental health
- High School diploma or equivalent with a minimum of five years of supervised experience in behavioral health
- Prior experience as detailed above
- Traveling within the following counties: Madison, Calhoun, Jersey, Greene, Macoupin, Montgomery, and Bond
- Conducting in-person, community-based child and family team meetings
- Reviewing IMCANS
- Supporting crisis services
- Evaluating prevention and safety plans
- Works in consultation/collaboration with clients, family members and other providers to implement plan
- Provides assertive outreach, advocacy, support and follow-up to assigned clients
- Documents all outreach attempts
- Ensures all services/outreaches indicated in the personal growth plan are provided in accordance with identified frequency
- Provides ongoing case management services for assigned service recipients, including but not limited to assessment, planning, linking, monitoring, referral and advocacy
- Provides case management to Family Support Program (FSP) and/or Specialized Family Support Program (SFSP) youth
- Assists client in building a natural support team
- Integrates natural community supports/resources, for any and all life domain area(s) identified as needing improvement, into the personal growth planning process and service delivery process
- Presents relevant information for the purpose of assuring quality of care, gaining feedback, and planning changes in provision of personal growth-based services
- Identifies and reports all Critical Incidents according to Centerstone policy
- Provides community education as assigned
- Effectively communicate via written, verbal, in person and virtual methods
- Skill in customer service/experience techniques
- Knowledge of role and function of various community resources
- Skill in problem-solving
- Knowledge of basic service recipient's dynamics of assigned disability groups
