Job Summary
Care Coordinator for Case Management (Part-Time, Days) coordinates care and services for an assigned patient population across the episode of illness with the patient, family, care team, and payors. Utilizes advanced clinical nursing skills to assess, plan, intervene, and evaluate care, with emphasis on utilization management, discharge planning, and collaboration to optimize clinical and resource outcomes. Responsibilities include evaluation of quality/utilization cases, admission/discharge criteria, 1-day stays and 30-day readmits, potential assignment to the Emergency Department, advocacy and communication with patients and families, and serving as a resource to ensure efficient care progression and appropriate transfers. Requires two years of acute care clinical experience or a Master's in Nursing with a Case Management emphasis; current RN licensure in CA; strong analytical, critical thinking, and interpersonal skills; familiarity with managed care and hospital information systems; proficiency with clinical documentation and office software; and ability to maintain patient confidentiality. Required Qualifications
- Current licensure with the California Board of Registered Nursing
- Master's in Nursing with a Case Management emphasis OR two years of acute care clinical experience with case management experience
- Excellent communication and interpersonal skills; ability to collaborate with physicians and external agencies
- Knowledge of managed care requirements and hospital information systems
- Experience with Epic/Midas/Chart Max and Microsoft Office Suite (Word, Excel, Outlook)
- Ability to maintain confidentiality and advocate for patients and families
