Job Summary
Care Coordinator to Care Manager role in a high-acuity home care setting. Own the client experience from initial inquiry through ongoing care by building individualized care plans, conducting in-home assessments and supervisory visits, coaching caregivers, and coordinating with families. Responsibilities include client care & coordination, conducting in-home consultations, scheduling, caregiver support, and ensuring quality and accuracy in documentation and records. The role also covers inside sales/inquiry follow-up, leading operational workflows, and contributing to growth and retention KPIs. On-site presence in the East Bay service area with a warm, organized, proactive approach and the ability to manage multiple moving parts in a fast-paced home-care environment.
Required Qualifications
- 2+ years of experience in care coordination, client services, case management, or inside sales in home care or healthcare
- Excellent verbal and written communication skills
- Comfortable with technology, including WellSky (formerly ClearCare), Zoho CRM, and AI tools such as ChatGPT/Claude
- Resides in our service area and able to work onsite and drive across our territory
- Valid driver’s license, insured vehicle, and availability for on-call support
- Registered on the Home Care Aide (HCA) Registry, or able to complete registration (LiveScan background check, TB clearance, and required entry-level training)
