Job Summary

Identify children with special health care needs and conduct patient and family assessments to determine strengths, needs, and resources. Engage families in developing individualized Care Plans and coordinate their implementation through ongoing monitoring, periodic evaluation, and referrals to providers for enrollment or admission. Perform home visits based on family needs and provide education and support regarding medical conditions and available resources. Assist families in accessing respite care, extended services funds, and other community resources while maintaining accurate case files and strict confidentiality. Conduct data collection activities, attend staff and interagency meetings, and maintain collaborative relationships with healthcare providers and community partners.

Required Qualifications

  • Bachelor's degree in social work, nursing or other related field
  • Registered Nurse licensed in CT
  • BSN
  • Access to reliable transportation on a consistent basis
  • Valid driver's license
  • Auto insurance

Desired Qualifications

  • Bilingual English/Spanish
  • Experience with children and youth who have special health care needs and their families
  • Experience working as part of a multidisciplinary team
  • Demonstrated knowledge of treatment planning process
  • Strong problem-solving skills
  • Written communication skills
  • Verbal communication skills
  • Knowledge of community resources
