Job Summary
The Care Coordinator plays a critical role in managing and coordinating health services for children, primarily Medicaid recipients, ensuring they receive necessary care and support. Required Qualifications
- Bachelor’s Degree in Human Services and/or related field
- Bilingual Spanish speaker/read/write
- A passion for serving children and families with special needs
- Valid NYS Driver’s License
