Job Summary
Conduct in-person face-to-face assessments in Roanoke, VA, with remote work options available for days without scheduled assessments. Coordinate care transitions and service navigation for individuals with Intellectual and Developmental Disabilities, substance use disorders, and behavioral health needs. Advocate for vulnerable populations and support waiver services where applicable. Requires a Bachelor's degree in Health & Human Services or related field, one year of social work or care coordination experience, and full-time commitment (40 hrs/wk). Desired Qualifications
