Job Summary
Provide telephonic, virtual, and in-person support to individuals served, assisting with housing, employment, healthcare, and essential needs while accompanying them to appointments. Identify individuals in need through weekly outreach, evaluate their circumstances, and act as their primary advocate to ensure their voices are heard and autonomy is promoted. Serve as the point of contact for caseloads, identifying safety issues and taking appropriate measures in consultation with leadership. Secure and monitor services, develop individualized care plans with clients, and include appropriate parties in the Integrated Care Team. Build relationships with community partners, respond to requests for referrals, and complete accurate documentation and reports for funding agencies. Provide transportation for critical needs when vehicles are unavailable. Comply with program standards, productivity metrics, and funder contract requirements.
Required Qualifications
- High School Diploma or GED
- 1-2 years of work experience in related behavioral health, human services, and/or medical field
- Valid driver's license
- reliable, legally registered vehicle

- Bachelor's Degree
- Bilingual, Spanish and English speaking
- Demonstrates cultural and linguistic competence in working with individuals of varied cultural backgrounds and linguistic needs
- Demonstrates genuine care and concern for individuals served, acknowledging their struggles and validating their experiences
- Provides support without judgment, recognizing each individual's situation is unique and complex
- Pays close attention to individuals' concerns, actively listens, and responds with sensitivity and empathy
- Treats all individuals served with respect and upholds their dignity throughout the service delivery process
- Knowledge of and ability to utilize motivational interviewing techniques to help individuals identify strengths, set goals, and develop hope for the future
- Demonstrates willingness and ability to serve challenging and diverse populations
- Demonstrates working knowledge of community resources and how to help individuals access them
- Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance use, and care management
- Professional demeanor in communicating with individuals served, colleagues, and other providers
- Ability to communicate clearly and effectively with individuals served, providers, and staff both verbally and in writing
- Familiarity with Internet, email systems, and electronic health records (EHRs)
- Working knowledge of program KPIs and performance metrics and how they impact the role, the program, and the overall mission of the One Care program
- Provides exemplary and culturally sensitive customer service to individuals served
