Job Summary

Conduct initial and updated assessments, including CANS, CRAFFT, and diagnostic evaluations to guide care planning for youth and families. Convene Care Planning Team meetings to develop Individualized Care Plans and work directly with clients to identify strengths and strategies for meeting goals. Ensure Risk Management/Safety Plans are completed and updated, while collaborating with state agencies, schools, and community providers to coordinate medically necessary supports. Monitor caseload expectations, documentation, and service delivery against program standards. Prepare youth and families for transitions when goals are met and progress is sustained. This role requires a Master's degree, CANS-MA certification, and three years of behavioral health experience.

Required Qualifications

  • Master's degree in a Human Services-related field
  • CANS-MA certification
  • At least 3 years of experience providing behavioral health services to children, adolescents, and families
  • Knowledge of Serious Emotional Disturbance (SED) and child/adolescent treatment techniques
  • Strong facilitation, crisis planning, and collaboration skills
  • Valid Massachusetts driver's license
  • Reliable transportation

Desired Qualifications

  • Experience with Wraparound programs or home-based services
