Job Summary
Care Coordinator role within a Certified Community Behavioral Health Clinic (CCBHC) multidisciplinary team. Combines direct client care support with care coordination: intake, vital signs collection, care gap closure, hospital/crisis follow-up, referral coordination, social service linkage, and client engagement. Responsibilities include medical assistant tasks (rooming, vital signs, specimen handling, EHR documentation), care coordination (scheduling, reminders, follow-up on tests and referrals, outreach to address care gaps, high-risk patient monitoring), and collaboration with physicians, nurses, care coordinators, and community partners. Requires familiarity with EPIC/EHR systems, ability to communicate across care teams, and commitment to trauma-informed, client-centered care. Bilingual English/Spanish preferred.
Required Qualifications
- High school diploma or GED.
- Medical Assistant certification or relevant healthcare experience.
- Minimum of one (1) year of experience in healthcare, behavioral health, public health, or community health setting.
- Basic knowledge of medical terminology and client care procedures.
- Proficiency with Electronic Health Records (EHR) and Microsoft Office applications.
