Job Summary
Care Coordinator roles focus on helping individuals and families develop skills, access resources, and manage illness to promote recovery and wellness. Responsibilities include providing education to person(s)-served about mental illness and/or substance use disorder; arranging transportation as needed; linking person(s)-served to community resources and natural supports; helping develop interpersonal, social, family, community, and independent living skills; completing required documentation; adhering to evidence-based practice models; understanding diagnostic criteria for substance use and/or mental disorders; effective oral communication; ensuring program operations meet CARF/DMH standards; following agency policies; and handling other duties and special projects as assigned.
Required Qualifications
- A master’s or bachelor’s degree in a human services field from a college or university included in the US Department of Education’s database of accredited schools
- Qualified Addiction Professional (QAP)
- Associate of Applied Science degree in Behavioral Health Support as designated by DMH
- Bachelor's degree unrelated to human services field with two years of qualifying experience
- Four years of qualifying experience
- Qualifying Experience must include delivering services to individuals with mental illness, substance use disorders or developmental disabilities
- Experience includes one-on-one or group services with rehabilitation/habilitation and recovery/resiliency focus
- Experience teaching and modeling coping strategies for psychiatric/substance use issues
- Experience supporting housing, employment, or community functioning goals
- Experience helping individuals achieve goals on individualized treatment plans
